Negative association between right bundle branch block and inducibility of ventricular tachyarrhythmias in Chagas disease

چکیده
I read with interest the article by Cedraz et al.(1) describing their findings on the electrophysiological study of 115 chagasic patients. The authors reported that the presence of right bundle branch block (RBBB) was associated with less induction of sustained ventricular tachycardia or ventricular fibrillation (VT/VF) in a univariate analysis. In 56 patients with RBBB, VT/VF was induced in 35 (37.5%), versus 21 of 59 cases (57.6%) without this disturbance (p=0.03). This difference resulted in an odds ratio of 0.44 (confidence interval 95%; 0.21-0.93). Unfortunately, the authors did not discuss this finding. Some studies have shown the inducibility of VT/VF as a predictor of mortality in Chagas disease.(2-4) The presence of RBBB was already demonstrated as an independent predictor of mortality in a multivariate analysis.(4,5) Therefore, the negative association between RBBB and inducibility of VT/VF in chagasic patients is a unique observation that should not be disregarded and needs further investigation.

Electrophysiologic investigation in Brugada syndrome; yield of programmed ventricular stimulation at two ventricular sites with up to three premature beats.
INTRODUCTION Numerous reports on the inducibility of ventricular tachyarrhythmias (VT) in patients with atypical right bundle branch block and right precordial ST-elevation (Brugada syndrome) are based on multicentre studies that have used different stimulation protocols. Therefore, we prospectively investigated the inducibility of VT in these patients using a uniform protocol. METHODS In 41 ...

Left ventricular aneurysms (LVAs) can be congenital or acquired. They develop most frequently after myocardial infarction. Other causes include hypertrophic cardiomyopathy, arrythmogenic right ventricular cardiomyopathy, myocarditis, chest trauma, sarcoidosis or Chagas disease [1, 2]. LVAs without identifiable cause are considered as idiopathic. Most of LVAs are asymptomatic and are occasionall...
متن کاملArrhythmogenic right ventricular cardiomyopathy in boxers.
Arrhythmogenic right ventricular cardiomyopathy, formerly termed boxer cardiomyopathy, is a familial primary myocardial disease that is prevalent in boxers. Unique histopathologic changes in the myocardium lead to conduction abnormalities that typically manifest as ventricular tachyarrhythmias with left bundle branch block morphology. Affected dogs can be asymptomatic or may have syncope and/or...

Different polymorphic ventricular tachyarrhythmias may cause syncope or cardiac arrest in patients with no heart disease: (1) Catecholamine-sensitive polymorphic ventricular tachycardia (VT) presents during childhood: the hallmark is the reproducible provocation of atrial and polymorphic ventricular arrhythmias during exercise, despite a normal QT. Beta-blockers are the treatment of choice. (2)...
متن کاملElectrocardiography in Chagas' heart disease.
Conventional ECG still plays an important role in the overall knowledge of Chagas' cardiopathy, because of its importance in longitudinal and epidemiological studies, its diagnostic value, and its utility in prognostic evaluation. The authors discuss these aspects, as well as the use of eCG in the acute phase and the significance of a normal ECG in Chagas' disease. Correlations were made betwee...
